Hypertensive Crisis in a Patient With a Functioning Mesenteric Paraganglioma: Dramatic Response to Octreotide Treatment

Background/ObjectiveTo report a dramatic and immediate clinical biochemical response during treatment with octreotide in patient functioning mesenteric paraganglioma (PGL).Case ReportA 44-year-old woman was admitted severe hypertensive crisis blood pressure reaching 260/150 mm Hg. She 2 months postpartum had been previously diagnosed pre-eclampsia. Secondary hypertension suspected. This confirmed by finding 6 × 5-cm2 retroperitoneal mass located using 68-Gallium DOTA–octreotate positron emission tomography/computed tomography grossly elevated plasma catecholamine level of 93 000 pmol/L (normal reference range: 650-2433 pmol/L). Treatment immediately started high doses long- short-acting octreotide. After weeks before surgery, the normotensive, 120/70 Hg norepinephrine 6000 pmol/L. The tumor resection uneventful, histology diagnosis. Following remained normotensive without any medications.DiscussionPGLs pheochromocytomas are neuroendocrine tumors, most have receptors for case another reported responded dramatically to dose Earlier reports patients failing respond likely result smaller dose.ConclusionWe conclude that short- long-acting valuable severely patients. Our experience suggests is value other PGLs pheochromocytomas. rapid, sustained, effective, minimal side effects. To best our knowledge, this first functional PGL.
